SELF-DRIVE HIRE QUESTIONNAIRE

If you’ve already completed this form within the last twelve months, please complete a REPEAT FORM instead.

This form must be completed in full by both the Hirer (person who has paid for the hire) AND the Driver no sooner than 21 days before the hire date. No later than 12 hours before pickup. You are required to upload the following documents:

- A photocopy of the front and back of your Driver's Licence.

- Two proofs of address, no older than three months from the hire date. Bank Statement and/or Utility Bill will do.
